Participants for this study will include, newly diagnosed patients, Patients who previously initiated treatment, discontinued it midway, and are now resuming treatment, and all follow-up patients admitted to the psychiatric ward or attending the psychiatric clinic of the Teaching hospitals. This will also include patients who attend follow-up care at other regional psychiatric clinics after their initial recruitment in the wards .The diagnosis of patients will be confirmed by the written diagnosis on the Bed Head Ticket (BHT) or in patient notes/clinic books.
